



The Digital Health Scorecard is an easy-to-use app that asks seven key questions about your current health measures and habits. Based on your answers, your overall health score (0-100) is calculated.
The higher your score, the lower your likelihood of developing chronic conditions such as diabetes, heart disease or cancer. These types of chronic dieases account for 3 of every 5 deaths around the world!1

Most people should be able to get their health score in just a few minutes. The app saves your most recent score and even enables you to share a report with your healthcare professional.
The Digital Health Scorecard works great with touch devices like iPad, iPhone, Android and Windows Surface. For traditional desktop and laptop computers, try the Windows 8 app. Web-based browser version coming soon.



Don't know all your health measures? That's okay. The app is designed to guide you to your health score even if you don't know your exact numbers.
Of course, it is still a great idea to make an appointment with your healthcare provider for a simple blood test that can provide you with more precise information.
